Electronic forms are available for Vice Chancellors, Deans, and authorized faculty and staff. These forms may be accessed by clicking on the link below, and then clicking on "VC - Business Affairs" in the menu on the left side of the Electronic Forms page.
The Budget Adjustment Form is to be used by Vice Chancellors or Deans to transfer funds from their accounts to those departments reporting to them.
The Budget Revision Form can be completed on the screen; however, the form must be printed, sent on for approvals, and then submitted to the Budget Office.
